This program is designed to help Direct Support Workers (DSW) obtain valuable training
to help them provide the best care for their clients. DSWs who work for Self-Directed
clients are eligible to receive paid training through this program.

• • Participants Earn $17/ training hour.
  • The maximum training reimbursement is $550.00 per DSW, regardless of how many employers the DSW works for. Taxes are not levied on totals from any given source on totals less than $600, but the IRS does reserve the right to
    request tax reporting forms. SACK will provide documentation upon request.
  • Qualifying training classes are attached to this form.
  • DSWs are encouraged to take all trainings that apply to their role regarding their specific employer(s).
  • Reimbursement payments will be made when the DSW has completed all desired training and submits verification to SACK. Payment will be issued one time to any given DSW, so it is essential to only request payment after all desired trainings are completed.
  • Training Verifications and payment requests may be made at any time after June 15, 2026 up to December 15, 2026. Payments will be made by check mailed to DSW’s address. These checks can take up to two weeks to be mailed to DSW.
  • Reimbursement Payment Request must include:

    • Completion confirmation from training website of each module submitted for payment.
    • One to two sentence statement of how the training relates to DSW’s specific role.
    • Signature confirmation that all desired trainings are completed and this is the only request for reimbursement payment.
